Referring a patient to the hospital directly
Please download and complete the international patient referral form and complete a medical report . This should be in English and no more than three weeks old. Please give the patient's details including:

Visa assistance
Once your appointment is confirmed and funds have been received your international care co-ordinator will send you a 'confirmation to treat' letter. This will include all the details you need to help you apply for a visa if you need one.
International care co-ordinators are here to help and advise you. However, Bupa Cromwell Hospital has no influence on UK immigration policies. Securing an appointment and receiving an invitation to treat does not guarantee that your visa application will be successful.
Important financial matters
Self-pay patients
Once your appointment has been confirmed we will send you a letter giving details of the appointment and the estimated cost of our services. To guarantee your appointment, please complete and sign a credit card authorisation form before you arrive. Alternatively you can transfer funds directly to our bank account. As a self-pay patient you may receive separate invoices from your consultant and anaesthetist. If you have deposited sufficient funds with us, we can settle these accounts for you.
When you arrive at the hospital your international care co-ordinator will help you complete any financial matters prior to your appointment with us. If we need to schedule additional appointments we will prepare a detailed and personalised estimate for you.
Before you leave the hospital, a financial administrator will review all posted and estimated charges and payments with you. To ensure that your account is complete and accurate we will send you a final statement of charges within 20 days, if you need to have this information sooner, please tell you international care coordinator.

Insurance sponsored patients
We work with many health insurance companies throughout the world. If you have insurance cover, please give your international care co-ordinator the policy details when you request an appointment.
Please bring with you the letter of authorisation from your insurance company. If you are being admitted as an emergency patient, a financial administrator will contact your insurance company to confirm your eligibility, and seek their authorisation for your admission. If Bupa Cromwell Hospital does not have an agreement with your insurer, you can pay your account to us directly then submit a request for reimbursement directly to your insurer.
Please note that you are responsible for any co-pay, deductible and/or co-insurance amounts, as well as the cost of services denied for payment by the insurance company. Payment must be made in full before we can begin services. Please also note that many international insurance companies require their clients to pay for out-patient treatment and then seek reimbursement.
