MAKING AN APPOINTMENT

Step 1 - Send us your referral form and medical report

If you would like an appointment with any of the the Bupa Cromwell Hospital consultants please download and complete the referral form (PDF) and fax it with a Medical Report to the International Patient Centre:

If you would like help completing the form and are based in China, Cyprus, Libya or Pakistan, you can contact the international office in your area. They will get back to you within two to three working days.

 

Your medical report

The medical report should be from the doctor of the patient. It should be in English and should be no more than two to three weeks old. Ideally, it should contain the following information:

  • name
  • age
  • medical history to date
  • general fitness
  • reason for seeking treatment
  • other medical issues
  • any other relevant information.

If you prefer, we also have international officesin China, Cyprus, Libya and Pakistan who can help you with various aspects of your travel and treatment.


Step 2 - Review of your medical report, estimate of costs and appointment date

Once we have received your medical report and referral form we will forward it to the appropriate consultant. The doctor will review your case and we will provide you with an estimate for the cost of your treatment.

We will then send you an appointment date and advise you on approximately how long your treatment will take so you can plan your trip to the UK accordingly.


Step 3 - Final cost and confirming your appointment

We will send your appointment details along with a final cost estimate. It is important to understand that this is only an estimate. It is possible your costs may be more or less depending on the tests and course of treatment determined by your consultant once they have seen you. 

We will send you confirmation of your appointment which we you need you to return by phone, fax or email.


Step 4 - Assistance with your visa

Once your appointment has been confirmed and funds have been received your international patient care co-ordinator will issue you with a 'confirmation to treat letter'. This letter will include all the details you need to help you with your visa application process.
 
The international patient care co-ordinators are here to assist and advise you throughout the visa application process. However Bupa Cromwell Hospital has no influence on UK immigration policies. Securing an appointment and receiving an invitation for medical care does not guarantee that your visa application will be successful.
